Please Note: Some providers at Rooted Counseling Services accept insurance, while others do not. Please check the individual Providers page for more details on accepted insurance policies and self-pay rates.

Out of Network Provider

If your insurance is not listed above, Rooted Counseling Services would be considered an out-of-network provider. You may request an appointment receipt to submit to your insurance company for potential reimbursement based on your specific benefits.

We recommend asking these questions to your insurance provider to help determine the possibility of reimbursement for out-of-network coverage :

  • ​Does my health insurance plan include mental health benefits?

  • Does my health plan cover out-of-network providers?

  • Do I have a deductible? If so, what is it, and have I met it yet?

  • Is there a maximum number of therapy sessions allowed under my current health plan?

  • Do I need written approval from my primary care physician for services to be covered?

Payments

All payments will be collected by credit card (most major credit cards or HSA cards) at the end of each visit. The credit card information will be stored electronically using HIPAA-compliant medical records software. Cancellation Policy

We require 24-hour notice to cancel a scheduled appointment. In cases of less than 24-hour notice of a cancellation, or of a missed appointment, a client will be required to pay the full cost of the missed session. This policy may be re-considered in cases of emergency. *During the summer and holiday months, we have a 48-hour cancellation policy.
